He was predeceased by: his parents, Ronald Ulaszek and Diane Ulaszek; and his mother-in-law Shirley Kulisz. He is survived by: his wife Laura Ulaszek (Kulisz); his children, Andrew, Nathan and Hannah; his siblings, Keith Ulaszek (Stacey), Stacy Demeter (Joe) and Matt Ulaszek (Val); his father-in-law David Kulisz; his brother-in-law Ken Kulisz (Vanessa); his nieces, Samantha and Matilda; and his nephew Ricky.
