He was predeceased by: his parents, Emily and Jerome; and his brothers, Richard, Robert, Dennis and John.

He is survived by: his wife Donna Puchalski (Nee Soborowski); and his stepsons, Jason Siembida and Mike Siembida.

Memorial donations may be made in Dan's memory to St. Jude Children's Research Hospital.
