She is survived by: her husband Thomas P. Addis; her children, Michael Koluch (Claudia), Kristin Roy (Olivier) and Kenneth Koluch (Jennifer); her grandchildren, Denisse Lopez Camacho, Adam Koluch, Josephine Koluch, Benjamin Koluch, Kara, Leah, Lauren Reese and Evangeline Roy; and her brothers, Frank Pavelek (Stella) and Bill Pavelek (Cindy). She is also survived by nieces, nephews, many friends and extended family.
