He is survived by: his wife Joann Sullivan; his son Theodore Sullivan; his stepsons, Mark Moore, Christopher Moore and Matthew Moore; his siblings, Janette Appley, Janice Crabb and Richard Dake; and his grandchildren, Jessica Sullivan and Brandon Sullivan (Brianne).
