He is survived by: his parents, Melissa Chapman (Mark) and David Crook II (Anne); his brother Nathan Crook (Cass); his grandparents, Beverly Wise (late Ken) and David Crook (late JoAnn); and his nephew Benjamin Crook. He is also survived by several aunts, uncles, and cousins.
