He was predeceased by: his parents, Charles Cuykendall and Arleen Cuykendall (Colton); his mother-in-law Shirley Jean Culberson (Mackey); his siblings, Sharon Kruse, Diana Phelan, Charles Cuykendall, Gerald Cuykendall, Sr., Robert Cuykendall and Dale Cuykendall; and his brother-in-law Arnold Bruck.
He is survived by: his son James David (Beth); his wife Deb; his daughter Amber Sivil (Jimmy); his grandchildren, Mark "Hollywood", Jake, Jana "Sis", Beyonca of Joey, AJ and Daniel; his siblings, Paulette "Sue" Smith (Chuck), Lois Bruck, Alice Bashaw (Jack), Tammy Green, Duane, Kenny Cuykendall and Brandy Hamilton; his father-in-law Roger Mackey (Margaret Ann); his siblings-in-law, Dawn Wood (Jamie), Sherry Cuykendall, Keith Kruse and Greg Mackey (Kim); and his grandson Tyler Weaver. He is also survived by many friends and family.
