He was predeceased by: his parents, Curtis Dise and Amanda Dise (Monday); his siblings, Marie, Raymond, Roy and Juanita; and his grandson Jacob Zubkoff.
He is survived by: his daughters, Donna Theobald (Brad), Sandra VanEeckhoutte (Andy Compher), Monica Coulardot (Neal) and Tonia Stibbe (Alex); and his brothers, Clayton Dise (Carol) and John Dise. He is also survived by twelve grandchildren; seven great grandchildren.
