He was predeceased by: his parents, Clio Church and Mary Church (Phillips); his son Todd Church; his daughter-in-law Shannon Church; and his brother Gale Church.
He is survived by: his wife Kay Stearns; his children, Jeff Church, Debbie Miller (Lee) and James Church (Sheila); his grandchildren, Camryn, Alicen (Josh), Ari (Dom), Breana, Landon, Kennedy and Owen; his great grandchild Jaxon; and his siblings, David Church, Loretta Jones and Dennis Church (Lynn). He is also survived by other loving family members and friends.
In lieu of flowers, memorial contributions may be made to Alzheimer's Association, in Phillip's memory.
