He was predeceased by: his parents, Paul Usiak and Irene Usiak (Clark); his daughter Marsha Lillich; his grandson Kyle Usiak; and his brothers-in-law, Jim Near and Richard Page.
He is survived by: his wife Barbara Page; his sons, Michael Usiak (Becky) and Mark Usiak (Deanna); his grandchildren, Ryan Usiak (Brecken), Katie Hillman (Ryan), Karyna Pochyla (Jason), Timothy Usiak (Julia), Lauren Lillich and Peter Lillich; his sisters, Theresa Stone, Donna Near and Sheryl Sallgren (Ray); his sister-in-law Carol Page; and his son-in-law Rick Lillich. He is also survived by nine great-grandchildren.
