He is survived by: his wife Michelle Sullivan; his children, Brandee Sullivan and Michael Sullivan; and his grandchildren, Nicholas Sullivan (Chelsea Krhovsky), Breyanna Warner and Johnathin. He is also survived by other loving family members and friends.
