She was predeceased by: her parents, Charles Hazen and Gladys Hazen (Miller); her husband John Paul Makowski; her daughter Stephanie Anne Makowski; and her brother C. Melville Hazen.
She is survived by: her children, Stephen Gerard, Michele Sivley (Jeff), Melissa Makowski (Gerald) and Mike Makowski (Vicki); her grandchildren, Rachel, Melissa, Clayton (Cheyenne), Stephanie, David, Ericka, Emily (Jarod) and Ethan; and her great grandchildren, Cayla, Haylie and Clayton.
