He was predeceased by: his sister Paige Elizabeth Mazurek; and his grandparents, Albert Thomas Jarvis, Dorothy Jeane Jarvis, John Henry Mazurek and Helen Lucille Mazurek.
He is survived by: his parents, John Nolan Mazurek and Donna Jeane Mazurek (Jarvis); his son Jon Wayne Mazurek (Brittany Myers); his girlfriend Brittany Myers; his siblings, Morgan Mazurek (Dave), Mullins, Brittany Hicks (Roger) and Jenna Weatherly (Ed); his grandparents, Sandra and Tom Linn; and his nieces and nephews, Gabby Mullins, Adelle, Alaina, Skylar, Starlar Hicks, Brinley Weatherly, Killian Edwards, Summer and Jaxon Coffman. He is also survived by many aunts, uncles and cousins.
In lieu of flowers, the family requests that donations be made to assist in setting up a college fund for Jon's son, Jon Wayne.
