He is survived by: his parents, James Haisma and Kimberly Haisma; his siblings, James, Theresa Haisma Jr. and Leslee Haisma; his nieces, April, Aubrey and Addison Haisma; his grandmother Jacqueline Bailey; and his cousins, Nolan, Asher and Gracie. He is also survived by many extended family members including aunts, uncles.

In lieu of flowers, donations may be made in Jay's memory to the Down Syndrome Association of West Michigan or Special Olympics Michigan-organizations close to his heart.

“Jim and Kim. I am so sorry about Jay. Nolan's heart is broken. Jay was always so sweet and polite to me. He will be missed by many but not as much as the two of you. It is so difficult to lose a child no matter their age. I can only say the memory never goes away and the heart will always hurt but with time it all just becomes who you are and life will go on just a little different. My prayers are with you guys as you wrap your head around Jay's passing and know he will be missed.”
