He was predeceased by: his parents, Chester A. Kozak and Ethel "Pete" Kozak (Reed). He is survived by: his fiancée Nancy Jean Hauck; his sisters, Dawn Mahoney (Dan) and Christy Hoffmann (Robert); and his siblings-in-law, Larry Hauck (Jan), Judy Jones, Alan Hauck (Lois), Karen Schiesswohl (Eric) and Helene Pavlov (Michael). He is also survived by many nieces and nephews.
