He is survived by: his wife Donisha Leonard of Sherwood; his children, David Otis Jr. (Leonard) and Michelle Leonard Smurr (Zachary) of Union City; his grandson Spencer Allen Smurr; his sisters, Tammy Harker (Joe) and Kimberly Keller (Bill); and his pet Otis Ramon.
