He was predeceased by: his father Gerald, Sr. Cook. He is survived by: his mother Shirley Cook (Powers); his brother Jerry Cook (Ellen); his nieces and nephews, Andrew, David, Amelia and Rebekah; his great-niece Lidoinet D. Cook; and his pet Sqeakers.
