He was predeceased by : his parents, Gerald Kuziak and Shirley Kuziak (Barnhill); and his brother Greg. He is survived by : his wife Stephanie Kuziak (Baker); his daughters, Heather Wallor (Justin) of Nashville, TN and Melissa Steele (Dakoda); his grandchildren, Hunter and Ryan; and his friend Stephanie.
