He was predeceased by: his parents, Iyler Cook and Marjorie Cook; and his brother Phillip I. Cook. He is survived by: his sister Kathleen S. Sapikowski; his brother-in-law Robert J. Sapikowski; his nieces, Connie, Karen, Margie and Becca; and his nephew Mike.
