He was predeceased by: his parents, Lawrence John and Theresa Jacobsen; his wives, Dorothy "Dottie" Jacobsen and Mariann Jacobsen; and his sisters, Dolores Jacobsen and Sharon Jacobsen.
He is survived by: his daughters, Deborah Duncan (D. R.), Dr. Theresa Peters (Jim), Dawn Craig (Bob) and Marilyn Jacobsen; his grandchildren, Kaitlin Duncan, Trevor Zalewski, Morgan Duncan and Brendan Zalewski; and his sibling Shirley Stepek (Bill).
Memorial donations in Jake's name may be made to the Church.
