He was predeceased by: his parents, Henry Smith and Priscilla Smith. He is survived by: his wife Donna Carnes; his children, Lee Smith (Courtney) of Hartford and Amber Pokorny (David); his grandchildren, Caydence, David, Sofia, Lainey, Emma, Theo, Valerie and Calvin; and his siblings, Dede Blinkman (Al) of New Auburn, Wis, JoLynn Kendzora (Dennis Camp) of Staunton and David Smith (Yvonne) of Sister Lakes.
Memorial donations may be made to St. Jude Children's Hospital.
