He was predeceased by: his daughter Bonnie Forman; his parents, Edna Forman and Barney Forman; his sister Noreen Forman; his parents-in-law, Sarah and Charles Deitch; and his brother-in-law David Deitch.
He is survived by: his wife Renee Forman; his children, DeAnna Forman, Jayme Forman and Charlie Barr; his grandchild Jordan Gray; his brother Walter Forman (Beverly); and his brothers-in-law, Jeffrey Deitch (Marsha) and Howard Deitch (Melissa). He is also survived by many nieces, nephews, great-nieces and great-nephews.
