He is survived by: his children, Ryan Cook (Shannon) and Steven Cook (Amy); his grandchildren, Damien (Ashtyn), Haylee (Arie), Nathan (Kailah), Aniyah (Tristan), Breahanna (Tyler) and Steven; and his great grandchildren, Waylon, Colbie, Malcom, Camille and Nora. He is also survived by his brothers and sister; and many nieces, nephews, extended family members, and friends.
