He is survived by: his wife Donna; and his children, Tim (Aoh), Michael, Amy Kohout (Mark), Laurie Olsen (Gary), Patrick, Katie Maday (Chuck), Anthony (Rebecca) and Molly. He is also survived by 14 grandchildren; nieces, nephews, relatives and friends.
