He is survived by: his siblings, Ronald (Barb), Fred, Lee and Jody; his children, Jack Jr. (Elisabeth), Denise (Matt), Wendy and Carlie; his nieces, Jenna and Rosie; and his grandchildren, Jack III (Kellie), Rebecca (Daniel), Daniel (Taylor), William (Kaylee), James (Karena) and Gregory (Breanna). He is also survived by 12 greatgrandchildren.
In lieu of flowers or formal services, memorial contributions may be directed to Blood Cancer United to support research dedicated to finding a cure for Leukemia and other blood cancers.
