He was predeceased by: his parents, John S. Clark and Dagny Braa Clark; and his brother Master Mason (A. F.). He is survived by: his son John S Clark; his wife Ann Forrest; his sister Connie Brauer (Joseph); his sister-in-law Moira Forrest (Brenda Boogren); his nieces, Brynna, Liberty, Jessie and Anne; his friends, Denette, Bill, Lyle, Brenda, Vickie, Annette, Steve, Ivan and Tina; and his caregivers, Patty, Dr. Timothy Kleindschmidt, Dr. Jessica Krog-Breeuwer, Dr. Darin Ruanpeng, Dr. Michael Stellmacker and Dr. Anna Perelshteyn.
