She was predeceased by: her husband Robert Formanack; her daughter Bonnie Newton; her great grandchild Haddie May Sauter; her parents, LeRoy Wagner and Mabel Wagner; and her siblings, Donald Wagner, Curtis Wagner, Robert Wagner, Wayne Wagner, Eldon Wagner and Ileta Larson.
She is survived by: her children, Bernadette Rodgers, Robert Formanack (Deborah), Bennett Formanack (Michaelene), Rebecca Sandquist (Thomas) and Barry Formanack (Carol); and her brother Charles Wagner (Patty). She is also survived by 18 grandchildren, 40 great grandchildren.
