She was predeceased by: her parents, Walter Formo and Ethel Formo (Proctor); her husband Richard; her siblings, George "Bill", Paula "Leanne" and Barry; her daughter Suzanne; her grandson William; and her great grandchild Ava.

She is survived by: her children, Deborah, Denise, Michael, Steven, Matthew, Timothy Kathleen, Sarah and Daniel. She is also survived by many grandchildren, great-grandchildren, nieces, and nephews.
