He was predeceased by: his parents, Grace Cook and Joseph Cook; his daughter Nicole Cook; his siblings, Bernie, Lyle, Duane Cook and Lorraine Pullis (Cook); his wife Donna Cook (Avis); his grandson Corey Miller; his siblings-in-law, Ronald Randall, Vonnie Cook, Via and Sheryl Avis; and his nieces and nephews, Roger Cook, Tom Pullis, Sharon Cook and Patricia "Trish" Smith (Pullis).
He is survived by: his children, Rhonda Waggoner (David), Kenneth Cook Jr. (Kacey), Lesa Tirado (Jose) and Shawn Cook (Jessica); his sister Dolores Randall; and his brothers-in-law, Don and Dick Avis. He is also survived by eleven grandchildren; and eleven great-grandchildren; and many nieces and nephews.
