He was predeceased by: his parents, Manfred Folstad and Tillie Folstad (Dybing); and his brothers-in-law, Stuart Johnson and Howard Nelson. He is survived by: his wife Collette; his daughter Michelle Mack (Rich) of Eden Prairie, MN; his granddaughter Addison Mack; his sisters, Ione Johnson of Moose Lake, MN, Nancy Nelson of Shelly, MN and Helen Bernhardson (Chuck) of Shelly, MN; and his siblings-in-law, Arnie Buhr (Barb) of Buffalo, ND, Connie Sletteland (Ray) of Devils Lake, ND and Cloyce Voeller (Jim) of Lake Elmo, MN. He is also survived by many nieces and nephews.
