He is survived by: his children, Terrence "Terry" Campbell of Lawrence, Neal Campbell of Olathe, Demi, Zack and Jordan Pavek; and his wife JoAnn. He is also survived by his sons and their families, his stepdaughters.
In lieu of flowers, the family suggests donations to the Midwest Fetal Care Center of the Children's Minnesota Foundation or the Michael J. Fox Foundation for Parkinson's Research.
