He was predeceased by : his parents, Dr. Joseph Kubicek and Lulu Kubicek (Bele); his sons, Wade and John (Sylva); and his children-in-law, Arthur Moe and Dr. Marilee Mead.

He is survived by : his son James; his granddaughters, Kathryn and Meredith; his daughter Kathryn Moe; his grandchildren, Elizabeth and Andrew (Sarah); and his great grandchild Salma Moe.

Memorials are preferred to the Lake Crystal Zion Lutheran Church Scholarship Fund or the Salvation Army.
