She was predeceased by : her former life partner John L. Formo; her son John Formo; her grandson Shawn Dodge; her great grandchild Maxwell Lindahl; and her brothers, James Belt Jr. and Donald Belt.
She is survived by : her children, Karen Dodge of Laporte, Carol Eisenbarth of Laporte, Tim Formo (Tania) of Laporte, Nancy Schmidt (Steve) of Laurel, MT and Penny Olson (Mike) of Laporte; and her sisters, Myrna Colliver of Edmond, OK, Valerie Parish (Roger) and Russell Belt (Karen) of Laporte.
