He was predeceased by: his parents, John Ptak and Ruth Ptak; and his brother Patrick Ptak. He is survived by: his siblings, Michelle Luzaich (John), Michael Ptak (Lori) and David Ptak (Betsy); Troy Isensee (Amanda), Stephanie Ptak, Kirsten Ptak (Andrew Montgomery), Carter Ptak and Bennett Ptak.
