She was predeceased by: her parents, Thomas Kotek and Estelline Kotek (Kukacka); her brother Glen Kotek; and her siblings-in-law, Al Viskocil and Mary Kotek.
She is survived by: her husband George Joseph Mader, Jr.; her children, Michelle Weiers (Dan) of New Prague, Sandy Johnston (Dale) of Montgomery, Sheila Janovsky (Tim) of Montgomery and Todd Mader (Theresa) of New Prague; her grandchildren, Eric Weiers (Jillian), Justin Weiers, Jenny Johnston, Nick Johnston (Shannon), Allie Gangloff (Jason), Elizabeth Johnston, Jason Janovsky (Melissa), Michael Janovsky (Allie), Jacob Mader (Hailey), Anthony Mader (Avery) and Joshua Mader; her great grandchildren, Joe, Trent, Travis, Trevor, Luke, Lillian, Robert, Sophia, Mackenzie and Jackson; her siblings, Ernie Kotek of Montgomery, Darlene Dietz (Donald) of Montgomery, Larry Kotel (Julie), Doreen Kuchinka (Larry) of Montgomery and Rosie Viskocil of Montgomery; and her sister-in-law Donna Kotek.
