He is survived by: his wife Denice (Durkot); his sons, Michael Conrad (Tammara) and Patrick Conrad (Kristin); his grandchildren, Natalie Conrad, Sean Conrad (Stephanie), Brooke Paquin (Mitchell) and Mallory Conrad; and his great grandchildren, Camden Paquin, Scout Conrad, Sloan Conrad, Bernard Paquin, Archibald Paquin and Walter Paquin.
In lieu of flowers, the family requests donations in Jerry's memory to be made towards St Jude's Children's Research Hospital.
