He was predeceased by : his wife Peggy Koktavy (Margaret Jones); his parents, Joseph F. Koktavy and Agnes E. Koktavy (Kotek); and his parents-in-law, Bert G. and Florence B. Jones (Derbort).

He is survived by : his grandchildren, Mikey and Joel; his children, Colleen (Jay), Johnson (Terry), Mike Jay, Pat Jay (Lori), Shannon Jay (Cathy), Brian Jay, Brendan Jay (Jackie) and Terry Jay; his siblings, Joseph G. Koktavy (Rosie), Milosh S. Koktavy (Bernita), Marcella Richard Nelson (Koktavy) and Edward A. Koktavy (Arlene); his siblings-in-law, Irene R Lien (Jones), Janet M. Timmerman (Jones), Robert R. Jones, Mary C. Marshall (Jones), Zita A. Nolan (Jones), Marilyn J. Jones (Hunt), Tom Nolan, Dan Timmerman and Clarence Lien; his daughter-in-law Becky Fahleen; his niece Erin Lien; and his nephew Joseph J. Koktavy. He is also survived by his Grandchildren and Great Grandchildren.
