She was predeceased by: her parents, William Hardtke and Anna Hardtke; her husband Luther Polzin; her former husband Lowell Urban; her siblings, Charles Hardtke and Mary Pedersen; her son Terry Polzin; and her grandson Scott Doherty.
She is survived by: her children, Alan Polzin (Susan Kirk) of Chatfield and Diane Holzer of Plainview; her grandson Kevin Doherty of Owatonna; her great grandchildren, Cohen and Aedan Doherty; her granddaughters, Chris Hayek (Phil) of Dexter, MN and Becky Wyness (Laura) of Hollywood, FL; her great grandchild Adelaide Wyness; her granddaughter Jessica Pate (Brian) of Ludowici, GA; and her stepchildren, Dale, Danny, Barb and Len. She is also survived by her grandchildren, and great-grandchildren.
The family kindly requests donations be made to the American Legion of Eyota or the Evergreen Cemetery of Dover in Annette's memory.
