He was predeceased by: his wife Heather; and his grandparents, Adolf, Alice Kolarik, Charles "Chuck" and Kay Johnson.
He is survived by: his parents, Lawrence Kolarik and Roberta Kolarik (Ann) of Waite Park; and his siblings, Michael Kolarik, Mark Kolarik (Pat), Steve Kolarik (Mary), Scott Kolarik (Karen), Mary Lynn Panek, Kathy Hage (Jeff) and Heather Benson (Mike). He is also survived by nieces and nephews.
In lieu of flowers, memorials are preferred to the Tri-County Humane Society in St. Cloud or the American Diabetes Association.
