She was predeceased by : her parents, Benjamin Hoffman and Mabel Hoffman (Dvorak); her husband William Hlavka; her sons, Michael Hlavka and Frank; her parents-in-law, Joseph and Rose Hlavka; her children-in-law, Lawrence Nowak and Therese Hlavka; her siblings, Benjamin Hoffman Jr. and Ramona Wosmek; and her siblings-in-law, John Schultz, Otto Wosmek, Joseph Hlavka, George Cullen, Sister Rosalyn Hlavka, Adelaide Cullen, Mary Hoffman and Elaine Hlavka.

She is survived by : her children, William Hlavka (Diane) of Silver Lake, Kathryn Nowak of Hutchinson and Susan Smykalski (Chuck) of Silver Lake; and her siblings, Vernon Hoffman (Marcella) of Phoenix and Nora Vollroth of Silver Lake. She is also survived by many other relatives and friends.
