She is survived by: her husband Don Vacek; her children, Scott Formo, Jeff Formo (Nadine) and Sharon Friedrichs (Jeremy); her stepsons, Brian Vacek and Paul Vacek (Lisa); her grandchildren, Coty Friedrichs (Breanna), Anthony Formo (Naomi), Brett Formo (Steph), J. R. Formo (Alexis), Caleb Formo (Kassy), Dalton Friedrichs, Jade Vacek and Autumn Vacek (Emilio); her great grandchildren, Aubree, Blair, Lincoln, Ellie, Jett, Levi, June, Ames, Ryatt and Irelynn; and her brothers, Raymond Larson (Shelly) and Mark Larson (Linda). She is also survived by nieces, nephews, other relatives, and friends.
