He was predeceased by: his parents, James Jr. Forman and Cecelia Forman (Ott). He is survived by: his wife Nancy; his daughters, Catherine Jacobson (Myron) and Rebecca Forman (Kyle Hammond); his grandchildren, Quinn Jacobson, Lydia Jacobson and Bryn Hammond; and his brother Steve Forman (Michelle).
