He was predeceased by: his parents, Hal Forest and Ruth Joyce Leazenby (Cox). He is survived by: his sons, John Leazenby (Ashley) and Cole Leazenby; his grandchildren, Madelyn, Natalie, Peyton, Alec, Ryker James, Grayson and Henry Banks; and his siblings, Richard Leazenby (Cathy) of Iowa City, IA and Janet Chapman (Doug) of Tucson. He is also survived by nieces and nephews.
