Alvarez (Jose) of Miami; and his grandchildren, Daniel Middleton, Derrick Middleton, Devin Middleton, Josie Stammer, Lucas Haverstick, Isabella Middleton, Nicole Barnes, Dorothy Casey, Britney Mumper, Amy Murdoc and Cody Brown. He is also survived by his eight great-grandchildren, numerous nieces, nephews, other relatives and many dear friends.
