He was predeceased by: his parents, Estle Anthony and Loretta Ellen Boehler Cook; his grandson Parker Cory Pugh; his niece Angela Hammond; and his siblings-in-law, Marsha Cook, Cindy Cook and Bill McElwain.

He is survived by: his wife Lori; his children, Allison Pugh (Cory), Rachel Oates (Eric) and Landon Cook (Kristina) of Butler, Missouri; his grandchildren, Brooklynn, Riley Pugh, Liam, Raquel Oates, Lincoln, Leelynn, Kordelia and Layton Cook; his siblings, Donald Cook, Barbara Morgan (Larry), Kenneth Cook and Sharon McElwain; and his pet Gem. He is also survived by nieces, nephews, cousins.
