He is survived by: his daughter Laura Cook (Wade) of Salem, Missouri; his grandchildren, Alexis Kulik (Martin) of Salem, Nathan Cook (Mackenzie) of Salem and Nathan Wolfe (Kelsey) of Licking, Missouri; and his great grandchildren, Aiden Mathess, Asher Carter, Arissa Carter, Rilyn Cook and Cooper Cook. He is also survived by nieces, nephews, other relatives and many friends.
