He was predeceased by: his parents, Dee Hayward and Margie Hayward (Martin); and his grandfather Orville Hayward. He is survived by: his wife Patti Hayward (Hartley); his children, Carrie Jean Hayward, Clinton Hayward (Emily) and Cary; his grandchild Madison Hayward; his brother Jim Hayward (Sue); and his uncle Russell Martin (Kate). He is also survived by relatives and friends.
