She was predeceased by: her parents, Leonard Winfield Houk and Etta Abigail Houk (Crowder); her husband Walter Hazel; her siblings, Wilbert Houk, Wilma Hardin, Carl Houk and Willard Houk; and her sons, Robert Carbah and Steven Carbah.
She is survived by: her daughter Teresa Phares (Chuck) of Clinton, MO; her grandchildren, Aaron Phares (Tiffany) of Springfield, MO, Amanda Lowry (Trevor) of Springfield, MO, Dean Carbah (Rachel) of Raymore, MO, Roberta Maisano (Christopher) of Waldwick, Stevie, Cheyenne and Cassandra Carbah of Mahwah, N. J.; and her great grandchildren, Amelie, Teslyn, George Phares of Springfield, MO and Vincent Carbah of Raymore, MO. She is also survived by numerous nieces, nephews.
The family suggests memorial contributions to St. Croix Hospice, and these donations may be left in care of the funeral home.
