He was predeceased by: his grandparents, Carol, John Jones, Jr., Betty, Paul Cook and Richard Mullaney; and his great-grandparent Buena Elizabeth Jones.

He is survived by: his parents, John Cook and Yvonne Carol Mullaney; his stepfather Jim Mullaney; his siblings, Melissa Jansen (Ryan), James Mullaney and Tony Cook; his grandmother Rita Mullaney; and his nieces and nephews, William Cook, Erin Cook, Callie Jansen and Ellie Jansen.
