He was predeceased by: his mother Loretta Sample (Barker); his wife Ann Sample; and his father Wayne Sample. He is survived by: his children, Megan Sample (Dakota) of Wayne, O. D, Sample, III, Kaitlynn Sample (Hunter) and Jacob Sample; his stepchildren, Sara Rarick (Daniel) and Andrew Eskridge (Mallory); his grandchildren, Dimitri and Naya; and his siblings, Kathy Leaverton (Eddie), Ricky Sample and Loretta "Cookie" Sample, Jr.. He is also survived by several nieces, nephews, cousins, other relatives and many friends.
