He was predeceased by : his father Raymond Boulware; and his brother Arnie Smith Jr.. He is survived by : his mother Bonnie Eads (Burnett); his children, Crystal Anderson (Ken) of Columbia, MO, Charolette Mae Miles of Columbia, MO, Randi Howell of Jefferson City, MO and Jairicia Gravitt (Gabe) of Boonville, MO; and his siblings, Angela Boyles, Debbie Barbey, Mary Oliver, Shannon Smith and Ryan Boulware. He is also survived by several nieces and nephews.

Family suggests memorials to the Woodville Cemetery (Memorials Payable to: Woodville Cemetery, 37587 State Highway Y, Jacksonville, MO 65260).
